Development of an interdisciplinary care plan that reflects key elements outlined in the current Conditions of Participation (CoPs) requires a patient-centeredness not currently optimized in many electronic medical record (EMR) systems. Frustration over “cookie-cutter” care plans is not a new phenomenon in the home health industry. To complete this component of care planning and coordination requires a fusion of all discipline-specific findings reflecting a “one beneficiary-one care plan” mindset. Key to any home health plan of care (POC) is the incorporation of measurable and meaningful goals expected as a result of skilled service(s) evidence-based/best practice interventions. These goals and interventions establish the framework for determination of discipline utilizations.
This webinar will provide attendees with clear strategies to assist clinicians in developing a patient-centered, interdisciplinary POC for a Medicare Part A home health episode of care. Practical application of care planning principles in the April webinar will be presented to address appropriate utilization of services that support the medical necessity of the care being provided. Case examples will include chronic disease management and aspects of both management and evaluation for nursing and maintenance therapy. Attendees will have the opportunity to review and assess common home care scenarios that contribute to assignment of nursing and therapy frequencies and duration.
