Development of an interdisciplinary care plan that reflects key elements outlined in the current Conditions of Participation (CoPs) requires a patient-centeredness not currently optimized in many electronic medical record (EMR) systems. Frustration over “cookie-cutter” care plans is not a new phenomenon in the home health industry. To complete this component of care planning and coordination requires a fusion of all discipline-specific findings reflecting a “one beneficiary-one care plan” mindset. Key to any home health plan of care (POC) is the incorporation of measurable and meaningful goals expected as a result of skilled service(s) evidence-based/best practice interventions. These goals and interventions establish the framework for determination of discipline utilizations.

Attendees will have the opportunity to review and assess common home care scenarios that contribute to assignment of nursing and therapy frequencies and duration.
