Process quality measures evaluate the rate of home health agency use of specific evidence-based processes of care. The standardized home health quality process measures focus on high-risk, high-volume, problem-prone areas for home health care. These include measures pertaining to all or most home care patients, such as timeliness of home care admission, immunizations, and use of risk assessment tools (e.g. for falls, depression). As well, there are measures for specific diagnoses (e.g. diabetes).

Session Description:
Often identifiable by “yes/no” response options, OASIS C2 items that create process measure data can cause anxiety in two key areas – is there documentation to support the selected response and the negative perceptions if “no” is selected. This session will assist data collectors and reviewers with making peace with both issues and ensure accuracy.
Session Objectives:
- Differentiate between a process measure and an outcomes measure.
- Define best practice and their relationship to OASIS process measures.
- Connect care planning to issues identified by process measures.
