If you say the word OASIS and someone visible flinches, it is a clear indication that you are talking to an individual who has worked in home health at some point in the last 10 years. The pursuit of accuracy in data collection has consumed enormous amounts of limited agency resources (time and money). Achieving a level of mastery, then sustaining those competencies, is not an easy task. Continued competency through education and training is essential to ensure knowledge remains current and accurate with changing guidance and data set updates.
Many clinicians and leadership staff sigh with relief once the OASIS passes internal review processes, the request for anticipated payment (RAP) is processed, allowing the episode of care to move forward. Too few clinicians have treated the OASIS as the “living document” it should be by not incorporating the data set information into care planning. This results in a disconnection between reported data and patient care delivery that creates challenges to outcomes and process measure management for the agency.
There are many ways to become educated in accurate OASIS data collection, including a freely accessible manual explaining every item in detail. It is one thing to understand the instructions, but another to conduct the assessment effectively to gather the required information. Knowledge application, a clear understanding of item intent, and score interpretation based on current guidance is critical. Once data is collected, integrating the information into the plan of care and the focus of every visit is necessary for consistent measurable outcome improvement.
